A case of Secondary Diabetes by Dr. Vinay Dhandhania

CASE OVERVIEW

In our daily practice of counselling, once in a while we come across unique cases that peak our interest. Dr. Vinay Dhandhania had such an experience when, a 42 year old female, known diabetic since 2014 visited him. She had complains of uncontrolled blood glucose levels, diminishing vision for past six months and weight loss; in spite of taking insulin daily, three times a day. Previous visits with other medical practitioners deduced the cause of diminished vision to be poor diabetes control.

DIAGNOSIS

Clinical examinations suggested features of acromegaly. Laboratory evaluation showed HbA1c of 10.4%, along with raised growth hormone (GH) and Insulin-like growth factor (IGF). Concurrently, the CT of brain was suggestive of pituitary tumor. Due to which she was diagnosed with secondary diabetes.

RESULTS

She was admitted in CMC vellore, for removal of pituitary tumor

TREATMENT

Post surgery, her glucose levels were well controlled with 30 units of insulin and OHA, metformin. Her vision improved markedly.
